Each HIP includes health coaching
and educational materials for high-risk enrollees and educational
mailings to low-risk enrollees. Eligible enrollees are identified
through medical and prescription drug claims, hospital discharge
reports, physician referrals if available and NCPPO
Medical Case Management. Participation is strictly optional.
“Health
Coaching” for Enrollees with Chronic Illness. By involving
enrollees in their own care management process through interaction
and participation, health coaching aims to reduce emergency
room visits and hospital admissions, reduce absenteeism,
increase productivity and promote proper use of the health
care system as a whole.
Asthma Health Improvement
Program. The Asthma HIP was developed to help enrollees better understand
and self-manage their conditions. Poorly managed asthma can
result in unnecessary visits to the hospital or emergency
room. Health coaches work closely with each participant to
avoid
these outcomes by developing an effective self-management
regimen, including trigger avoidance
and medication.
Diabetes Health Improvement
Program. Poorly managed diabetes can result in serious health
complications including heart
disease, stroke, blindness and kidney failure. These complications
significantly impact the costs associated with the disease,
which in the U.S. are nearly $100 billion a year. Diabetes
management is complex and often overwhelming. HIP dieticians,
registered nurses and exercise physiologists all play a role
in helping enrollees avoid health complications through effective
lifestyle changes. Regular outreach ensures that participants
get proper screenings and follow-up care.
Cardiovascular
Disease (CVD) Health Improvement Program. The CVD HIP was designed to provide
support to enrollees who suffer from heart-related diseases.
The program provides access to a multi-disciplinary team
of health coaches who help
enrollees live successfully with their condition. The CVD
HIP focuses on four cardiovascular
diseases – Congestive Heart Failure (CHF); Coronary
Artery Disease (CAD); Hypertension and Hyperlipidemia.
Musculoskeletal. With
the development of the musculoskeletal HIP a team of health
professionals including nurses, registered
dietitians, exercise physiologists and health educators
work together to provide the participate health coaching
techniques that support focused lifestyle changes that
can lessen the symptoms of the musculoskeletal conditions
including arthritis and lower back pain.
Oncology
Support. The Oncology Support HIP has been developed
to provide education and personal support to enrollees receiving
cancer treatments. The team of health coaches will assist
the enrollee with the management of their cancer, treatment
side-effects and other complications related to cancer.
Comprehensive
Health Improvement Programs combine wellness, prevention
and disease management
to guide enrollees’ and
physicians’ efforts toward improved quality of care,
a higher quality of life and better health outcomes. |
